Appointments

Your appointments with Linda will be educational, enjoyable and confidential. Each session will be personalized to your own individual health concerns, lifestyle, and nutritional needs. You will collaborate with Linda to develop short and long term goals and a personalized meal plan. Exercise and behavior changes will also be addressed.
It will be important that you are willing to make changes and follow Linda's instructions for success. To test your readiness for change, read this article.
Since new behaviors must be repeated several times before they become a habit, regularly scheduled follow-up appointments are encouraged to provide diet monitoring, continuing education and positive reinforcement.
Initial Appointment
On your first visit to Linda's office you will be asked to bring:
- A Food Diary for 3-5 days of typical eating
- A listing of your Medications, Vitamins and Nutritional supplements, and any Food Allergies or Intolerances
- A copy of your most recent laboratory Blood Work from your doctor, if available
- Your doctor's Referral Form or Diet Recommendations - not required
This initial appointment will last between 1 and 1 ½ hours. During that time Linda will...
- Have you sign a HIPAA Privacy Statement and Release of Information, if you want your nutrition counseling information released to other health care providers or family members.
- Complete a nutrition assessment
- Measure your body composition
- Determine your problem areas
- Develop short and long term goals
- Determine your calorie, protein and fluid needs
- Collaborate with you to develop an individualized meal plan
- Teach you the steps to follow for your particular diet success
- Provide detailed handouts
- Answer your questions
Follow-up appointments will last about 30-45 minutes. Be sure to bring your handouts and food diaries. During that time Linda will...
- Review your food diaries
- Discuss steps taken toward achieving your goals
- Review problem areas and set alternate goals
- Adjust the meal plan, if needed
- Answer your questions
- Praise your successes and bolster your weak areas
- Encourage you to plan positive reinforcement rewards
- Help you plan for diet relapses and other challenges
Food Diary
You will be asked to bring a 3-day food diary to your initial appointment. This should include what, when, and where you eat and drink. Amounts are not necessary. You may use the food diary format provided or you may develop your own format.
HIPAA Privacy Statement
This notice describes how protected health information about you may be used and disclosed. You will also be asked to sign a statement acknowledging that you have reviewed the notice carefully. You may download this form or receive one on your initial appointment.
Release of Information
You will only be asked to sign this form if you want your nutrition counseling information released to other health care providers or family members, in order to coordinate your medical care. If you are referred by a physician, you have already agreed to have the doctor and dietitian (Linda) coordinate your care. In this case, a release will not be required. Linda will not release your private information to anyone, except your doctor, unless you agree that she do so.
Payment may be made by cash or check (written to "Linda Farr") at the end of each appointment. Credit cards will not be accepted. Linda does not accept insurance and is not a Medicare or Medicaid provider. If you would like a receipt to turn into your insurance company for reimbursement, Linda can provide you with a HCFA 1500 form with the appropriate Medical Nutrition Therapy codes. You will need to ask your doctor or nurse for your diagnosis codes, to write on the form before you send it in.
Some insurance companies reimburse for Medical Nutrition Therapy, based on your particular medical diagnosis. In general, weight management alone is not a routinely covered benefit. Check with your insurance provider or review your own particular insurance policy and the options you have selected.
If you are a Medicare or Medicaid patient, you must seek the services of a Medicare or Medicaid provider, in order to be covered. Only specific nutritional diagnoses are covered under Medicare. In Texas, check with "Trailblazer" or your doctor for more details. Linda is not a Medicare or Medicaid provider.
